Uterine fibroids or myomas can affect fertility or chances of conception in case they become symptomatic. Myomectomy is a surgical removal procedure of myomas or uterine fibroids in women who wish to become pregnant.
Dr. Hrishikesh Pai, a leading IVF specialist and founder of Babies and Us Clinic one of the best IVF center in Mumbai, states, “Pregnancy after myomectomy is possible and should be embarked upon under proper medical supervision of an experienced IVF expert. Each patient requires a customized and evidence-based approach. Women can hope for a successful pregnancy experience. As an IVF specialist, I offer comprehensive treatment, support and care with the hope of delivering the best possible outcomes.”
The presence of uterine fibroids may or may not affect the chances of pregnancy or can create complications in pregnancy. Their presence can lead to various disorders such as pelvic pain, heavy and irregular bleeding, urinary and bowel discomfort, infertility, miscarriage, preterm labour, abnormal placentation, and post-delivery complications. Also, there have been instances of uneventful pregnancy despite uterine fibroids.
Gynecologists recommend medical treatment, embolization, myomectomy, and high-intensity focused ultrasound to treat uterine fibroids, the outcome of which depends upon the patient’s age and size along with the fibroids’ location and size. In cases where fertility preservation is a concern, options such as myomectomy or IVF may be discussed as appropriate methods to address uterine fibroids while considering the patient’s wish to experience motherhood.
There are three types of myomectomy surgeries: abdominal myomectomy, laparoscopic myomectomy and hysteroscopic myomectomy. Abdominal myomectomy refers to an open surgical procedure to remove large-sized fibroids located on the uterine wall, which cannot be removed through laparoscopic myomectomy and hysteroscopic myomectomy. Women who have undergone abdominal myomectomy should opt for a Caesarean delivery.
Laparoscopic myomectomy is a minimally invasive surgery through which the fibroids are pulled out using small surgical tools. It is considered a minor surgery, with the patient reliving the next day after a few hours of observation.
Doctors prefer a hysteroscopic myomectomy to remove submucosal uterine fibroids located in the uterine cavity. A hysteroscopic myomectomy is a scarless surgery as the hysteroscope, a telescopic tube, is inserted within the uterus to scrape the fibroids.
Dr Hrishikesh Pai suggests it is possible to get pregnant after myomectomy but after proper recovery and seeking adequate consultation for the same. Doctors recommend a gap of at least three to six months or more before attempting to get pregnant, as the uterus needs time to heal. Also, they indicate the need for a caesarean delivery instead of a normal delivery to prevent complications of a normal delivery.
Dr Hrishikesh Pai assures that patients need not lose hope despite being diagnosed with uterine fibroids, as it is possible to treat them through non-surgical and surgical approaches. Myomectomy has emerged as one of the most reliable treatment options to resolve fibroids.
