COVID-19 continues to disrupt essential health services in 90% of countries

GENEVA—The second round of a World Health Organization “pulse survey“ reveals that over one year into the COVID-19 pandemic, substantial disruptions persist, with about 90% of countries still reporting one or more disruptions to essential health services, marking no substantial global change since the first survey conducted in the summer of 2020.

Within countries, however, the magnitude and extent of disruptions has generally decreased. In 2020, countries reported that, on average, about half of essential health services were disrupted.In the first 3 months of 2021, however, they reported progress, with just over one third of services now being disrupted.

Overcoming disruptions

Countries have been working to mitigate disruptions. Many have now stepped up communications efforts to inform the public about changes to service delivery and provide advice about ways to safely seek health care. They are also triaging to identify and better meet the most urgent patient needs.

More than half the countries consulted say they have recruited additional staff to boost the health workforce; redirected patients to other care facilities; and switched to alternative methods to delivering care, such as providing more home-based services, multi-month prescriptions for treatments, and increasing the use of telemedicine.

In addition, WHO and its partners have been helping countries to adapt their processes so they can better respond to the challenges being placed on their health systems; strengthen primary health care, and advance universal health coverage.

“It is encouraging to see that countries are beginning to build back their essential health services, but much remains to be done,” says Dr Tedros Adhanom Ghebreyesus, Director General, WHO. “The survey highlights the need to intensify efforts and take additional steps to close gaps and strengthen services. It will be especially important to monitor the situation in countries that were struggling to provide health services before the pandemic.“

Persisting causes of disruptions

Countries are still having to make important decisions when responding to COVID-19 that may negatively affect access to care for other health issues. Redeployment of staff to provide COVID-19 relief and temporary closures of health facilities and services continue.

Although they may have taken on new staff, 66% of countries continue to report health workforce-related reasons as the most common causes of service disruptions. Supply chains are also still disrupted in nearly one third of countries, affecting the availability of essential medicines, diagnostics, and the PPE needed to safely and effectively provide care.

Communications efforts need to be further scaled up: more than half of countries report service disruptions due to patients not seeking care and because of community mistrust and fears of becoming infected.

Meanwhile, 43% of countries cite financial challenges as major causes for disruptions in service utilization. 

As a result, millions of people are still missing out on vital health care. In terms of services, the biggest impact reported by nearly half of countries is on provision of day-to-day primary care to prevent and manage some of the most common health problems. Long-term care for chronic conditions, rehabilitation, and palliative end-of-life care, is also still badly disrupted – severely affecting older people and people living with disabilities.

Potentially life-saving emergency, critical and surgical care interventions are still disrupted in about 20% of countries, reflecting the most immediate indirect consequences of the pandemic. Two thirds of countries also report disruptions in elective surgeries, with accumulating consequences as the pandemic is prolonged.

Among the most extensively affected health services (i.e. those for which more than 40% of countries are reporting disruptions) are those for mental, neurological and substance use disorders; neglected tropical diseases; tuberculosis; HIV and hepatitis B and C; cancer screening, and services for other noncommunicable diseases including hypertension and diabetes; family planning and contraception; urgent dental care; and malnutrition.

Issued ahead of World Immunization Week (which starts 24 April) and World Malaria Day (25 April) the survey reveals that serious gaps also remain in addressing disruptions to services in both these areas. More than one third of countries are still reporting disruptions to immunization services, despite progress in countries reducing disruptions to immunization services in health facilities and “outreach” immunization services by about 20% and 30% respectively compared to 2020. This highlights the need for new and sustained approaches to improving immunization coverage and uptake.

“The COVID-19 pandemic continues to pose serious challenges to global health beyond the impact of the disease itself,” said Henrietta Fore, UNICEF Executive Director. “For children, disruptions to immunization services have serious consequences. As we scale up delivery of COVID-19 vaccines, we have to ensure that this does not come at the cost of essential childhood vaccinations. We cannot allow today’s fight against COVID-19 to undermine our fight against measles, polio or other vaccine preventable illnesses. Prolonged immunization disruptions will have long-term consequences for children’s health. The time to catch up is now.”

Meanwhile, nearly 40% of countries are also reporting disruptions to one or more malaria services. While progress compared to 2020 – with about 10% fewer countries reporting disruptions to malaria diagnosis and treatment and 25-33% fewer countries reporting disruptions to malaria prevention campaigns (including distribution of long-lasting insecticide impregnated bed nets, indoor spraying and seasonal malaria chemoprevention), the reported level of disruption is still significant and needs to be urgently addressed.

WHO will continue to support countries so they can respond to increased strains on health systems and rapidly evolving priorities and needs throughout the course of the pandemic, and to ensure that COVID-19 control strategies are in balance strategies to tackle other health priorities and secure continued access to comprehensive care for everyone, including the most vulnerable.

Key support mechanisms include the ACT-Accelerator, which works to speed up equitable access to COVID-19 vaccines, tests and treatments, and the Strategic Preparedness and Response Plan, which guides actions taken at national, regional, and global levels to tackle COVID-19.

The Organization also remains focused on the delivery of the work it has committed to before the COVID-19 pandemic started. Internally, through the “Boost initiative” and the UHC Partnership, which covers 115 countries, WHO has strengthened its capacity to provide additional support to countries so they can maintain essential health services during the pandemic, and advance progress towards universal health coverage.

In 4 years he gained the confidence of his family, friends, and his town and he decided to be back in the field which was his dream but it was also a bit of a pickle.

Before for songs he used to go to different cities which were like 200-250 kilometers away from his city but because of covid-19 this came to an end and then he found a studio in his own city but it was not the appropriate one he wished for and moreover it was 5-6 kilometers away from his home, but this tough guy decided to go on anyhow. Aryan Arora said, ” I won’t cry for what I am lacking, but will use what I can”. The guys he’s working with right now are of his age and not professional in their work but Aryan says, ” you start, better to start with the ones who just started”.

Right now this guy is on the track to make himself among one of the big names in the industry. He has till now 7 songs uploaded on youtube and if you see, each song is better than the last one and right now he’s on a roll, all planned with 5 tracks to release in this and next month. Aryan is really a hard worker, working day and night for his dreams. Aryan said, ” I won’t sit here and cry that I can’t go outside, I will improve myself so that when everything’s fine, he’s gonna make himself shine like a diamond”.

Not Persuaded by PMLA proceedings against S.DilliRaj , Madras HC Quashes ED’s Complaint

Division Bench of Honorable Madras High court quashed the PMLA complaint No.70 of 2016 filed by  ED against S.Dilli Raj w.r.t default of bank  borrowings by First Leasing (FLCI)

As per the High Court’s order,  S.DilliRaj left the services of First Leasing(FLCI)  in 2007 and FLCI was making due repayments of its bank borrowings till September 2013.

The speaking order records that S.DilliRaj is not a beneficiary of Proceeds of Crime and even as per ED he received only Salary , Bonus & perks as an employee.

Observing that “ the salaries drawn by S.Dilli Raj can by no stretch of imagination be construed as Proceeds of Crime”, the High Court Bench held that it is not persuaded to agree with the Enforcement Directorate’s claim that that the Salaries & Perquisites that were paid to S.Dilli Raj while he was in employment with FLCI would amount to ‘proceeds of crime’ and any property purchased with that would stand tainted”.

Terming the impugned prosecution of Dilli Raj under PML Act as misconceived, the High court quashed the criminal complaint.

The primary charge against FLCI was that its accounts were fudged. S.DilliRaj neither prepared nor singed any financial statement like Balance sheet any time during his stint. Also, he was never a Director /Promoter/Shareholder /KMP of First Leasing.

Quash is the highest level of endorsement for one’s innocence. It is superior to Acquittal which follows trial with verification of all evidences and examination of all Witnesses. Discharge, another form of relief is also granted post-filing of the Charge sheet.  Criminal Complaint against S. DilliRaj is Quashed even prior to framing of charges.

After leaving First Leasing, S. DilliRaj joined SKS Microfinance in January 2008 and prepared SKS Microfinance  for its IPO which was subscribed 13.6 times. He played a key role in the team that turned around SKS Microfinance post the AP MFI crisis and repositioned the company into Bharat Financial Inclusion Ltd (BFIL) which was later merged with IndusInd Bank .

Pranjal Singh, a renowned film director from Bollywood has made a good name in the industry till now. Pranjal has made several National and International award-winning films. We got an opportunity to have a chat with Pranjal.

Were you the first bencher or backbencher in school days?

During School days I always tried to get a seat on the last bench. But most of the time I got 2nd last or third last seat. It was a competition with my friends to sit on the last bench. I have never sat on the first bench in my entire life. I was very much scared by mathematics. One thing I have been doing since my school days is I have never justified myself. Because I was loyal to myself. I used to think that if I am right then by no means I will justify myself, irrespective of what others think.

You have made several films on social issues. Which is one of the main issues you think worries you the most?

I’ve made several films about various social issues, such as tree conservation, water conservation, education, democracy, female molestation, to name a few. To date, every film I’ve made has dealt with extremely sensitive topics. Zindagi Ek Lamha is the film that I would prefer if I had to choose only one film that is closed to my heart. This film depicts how elderly people are ignored/neglected by both their children, youngsters, and society. The types of issues they face in old age homes once they get old are being ignored by everyone. It’s a film that will open people’s eyes about society and the children who abandon their parents during tough times.

It’s a bit personal, but how is your relationship with your parents? What was their first reaction when you told them you want to go into the film industry, and how did they react to it?

My relationship with my parents is very good. I was not confident enough that I would even pass my secondary exam or not. But my father always encouraged me and showed faith in me. And today in my family I am the most educated human being. As a father, he always wanted to see me working in the government sector as every Indian dad. But as a friend, he never discouraged me or felt guilty, when I was making films. He played both roles perfectly well in real life and whatever I have achieved to date, the credit goes to Father & Mother with their blessings.

Tell us about your struggle. Being an outsider it’s very difficult to survive and make a good name in the film industry. Did the outsider tag ever worry you?

There was a struggle, there is a struggle, and there will be a struggle until the last breath. If you accomplish anything without a struggle, you would not feel satisfied from the inside. I’ve been through the struggle before, and I’m going through it again today. It’s difficult to live here as an outsider. If I set my mind to accomplishing something, I will do everything in my power to make it happen. I am stubborn. I love doing work. I hate sitting free and doing nothing. During the night time also when I go to sleep I think if I can use this time to do some productive work.

Who is your dream actor whom you want to work within your film?

Currently, my dream actor is Shahrukh Khan. Earlier it was Irrfan khan. We had discussed one project which we would start in 2021, one film. But I am unfortunate that I was not able to work with him. I am now looking forward to working with Shahrukh Khan and also with Amitabh Bacchan.

What are your thoughts on nepotism?

One thing you first need to accept is Nepotism is everywhere. It’s not just limited to the film industry. You can see it in politics, the private sector, government sector & everywhere in the family also. If a father/mother is trying to help in his/her child’s career then will you call it nepotism? For me, the definition of nepotism is to kill someone else’s rights and give them to someone who does not deserve it. Like if a doctor wants to make his son a doctor, then you can’t say it’s wrong. This same rule follows for an actor and politicians also. But as I mentioned, if you kill someone else’s rights and give them to someone who does not deserve it then it’s wrong.

We have heard that you got several death threats, calls from goons, how you deal with it?

In my life I got many, I have always said to them the cost for one bullet is just 80 Rupees. Bullets are not having a name mentioned for whom it is. You can shoot me if you think I did any sort of crime. I will not stop making the right films. During the initial days, I was a bit scared but I thought I will not compromise with my work. I want to portray the clear-cut image of society that is my job, then I should not fear anyone. I tried to tell them my intention is not wrong. Whatever I am doing is the reality and you have to accept it. Some of them understood some not.

Well, Bihar is the hub of politics. And not asking you questions related to politics will be like disrespecting you not as a director but as a civilian who belongs to Bihar. How do you see the current situation of the country and Bihar?

Around 30-40 years back Bihar was having a great number of Raw materials. It was transporting it to several other states in India. But they were not importing anything from states. And once Jharkhand was formed then most of the industries shifted to their side. People of Bihar trapped themselves in religion-based politics and not in development-based politics. And the downfall of Bihar started in the mid-’90s. Bihar needs to work on literacy rate and employability. We have seen most IAS/IPS/IRS officers are from Bihar. But due to lack of resources, they never come back and migrate from Bihar. So the government must look to bring some industries and also to provide quality education to Bihar children. Bihar is one of the great states & really can do many things because it’s a growing state & I want to do many many things for the state, and many things are in pipeline.

Being one of the biggest film industries, Bollywood is not able to win an Oscar award. Do you think Oscar is overrated or Bollywood not able to produce Oscar-level films? Where do you think Bollywood is lagging?

Definitely not. You can’t say Bollywood is lagging. Bollywood is having great potential. The problem is, films that are made in Bollywood are made from a marketing point of view. In Bollywood, a film can survive only when it’s having great melodious songs, fight scenes, exceptional locations, and a big star cast. In the end, the main intention is to get the revenues. In India we have never seen cinema as an art, we make commercial films. The value of stories is a bit less and stars’ value is greater. It’s about the taste of the audience.

You are a good motivator and a great speaker. You have also given speeches in various Universities like Delhi University, Mumbai University, Jain (Deemed to be University), Symbiosis Institute of Technology, Amity University & Many More. How was your experience?

Great experience. I have also worked as a mentor and as a professor in various universities. I always thought that one day I will teach students who are in the media sector and management colleges. I want them to know what is happening in the actual industry. And my motive is to provide accurate knowledge of light, cameras, media, print, etc to the students. And as far as I know, I have worked with 23 colleges/institutes & universities. And I want to specially mention, during this pandemic I worked as a full-time teacher for around 10 months in one college. Students of this college were not able to receive a quality education. Along with this, the teachers were also facing several issues. Teachers were cheated for their salaries. The salary printed on the appointment letter and that was given in hand was having a huge difference. Salaries were not paid on time. They used to deduct money without any reason. This university was run with the support of one private company. The faculty were frustrated that too in covid pandemic time. Faculties were not provided with PF, Medical insurance, and various other facilities. Most of the faculty left the job in 10 days, 20 days, 50 days. And when I researched about this and I found this thing has been happening for the past 4-5 years. Ever since these private sector companies are involved in the education field, we have lost quality in education. I would like to request trusts, universities, to not do wrong with the future of students and faculties, else our nation will go back many years & will also request never connect with private consultancy in educations

What are your future plans in terms of films and for society?

After my past experience, I have decided to open a non-profit institute. This institute will provide a free or minimal amount of education to children belonging to poor families. I think “learning today is living tomorrow” should be the motto of our society.

And coming to films, I am looking forward to giving new technology to the film industry. I am working on a gaming film. Like if you are watching any film, let’s say Dabbang, and you want Shahrukh Khan in place of Salman Khan, then through this technology, you can change the actor. This will not take place as animation, it will take place realistically. & In the coming days, Immersive technologies will be used in this. We will be providing this technology to Nalanda university free of cost, the motto will tell the world about the history & the first resident university of India, Bihar, Nalanda, which functioned between the 5th and 13th centuries.

Meet Prathik Jain, a graphic designer who won Limca Book of World Record two times in a row

Can you picture artistic commercials when you think about graphic designing? Websites with attention-getting graphics? Beautifully laid-out spreads in magazines? It is because a designer knows that allowing oneself to make mistakes is part of creativity, knowing which ones to keep is part of the design. Hailing from Karnataka, Prathik Jain, is one such graphic designer who has set many world records with his incredible designing skills. 

Skill is the unified power of experience, intellect, and passion which Prathik has proved by creating a world record of ‘Most Number of Graphical Pictures’ on his Vespa scooter in just 2 hours which became India’s first graphical scooter. 

He travelled approximately 160 kilometres on his scooter from Tumkur to Bangalore for this modification. He was awarded the Limca Book of Records and the High Range Book of Records after completing the makeover.

Talking about interest in designing, he said “I believe that one of the most enjoyable aspects of becoming a graphic designer is being able to create something unique, images that have never been seen before. And the only way to do that is to stop doing what society tells you is fine. There is no such thing as nice. What’s good is what gets the job done.”

Not just he is an expert in designing but has an upper hand in software designing also. In just 12 hrs 51 min, he has developed 36 websites which is a record in itself. Limca Book of Records, High Range Book of Records, Unique World Records, Assist World Records, and Miracle World Records has all recognized Prathik Jain and his team.

Inspiring a lot of people, he tried his hands in business and started his own venture ‘Manueal,’ (www.manueal.com) Indian based men’s grooming company.

“Stop dreaming and start doing if you feel like there’s something you’re meant to be doing and you have a passion for it,” Prathik believes, making him special in his profession.
